List of Intake Forms

You can access the full list of intake forms from within the Smokeball desktop and web app at any time. 

Smokeball desktop app:

1. Select File from the top-left corner of Smokeball, then select Intake Forms.
   
2. Select the menu on the right-hand side of the Forms window to view all intake forms, all Lead forms or all Matter forms.
   
   
   • If your firm has created custom lead intake forms, these will also be listed when you only filter for All or Lead forms.

Smokeball Web App:

1. Select the gear icon on the bottom-left corner of the Smokeball Web App, then select Intake Forms.
   
2. Select the menu on the right-hand side of the Forms window to view all intake forms, all Lead forms or all Matter forms.
   
   • If your firm has created custom lead intake forms, these will also be listed when you only filter for All or Lead forms.
